This notice is to inform you of changes regarding your medical care. Dr. Annie Ashok will not be continuing to practice medicine at Sterling Ridge Orthopaedics and Sports Medicine (SROSM). Both Dr. Ashok and SROSM are dedicated to ensuring you receive the best possible continued care in regards to your medical condition and future orthopaedic medical needs.

SROSM is able to provide you with continued medical care. SROSM physicians and staff can visit with you at any of our locations. Should you choose to see another orthopaedic physician outside of SROSM, you may request a copy of all of your medical records at anytime by contacting SROSM and completing a medical records release form.
